Health Check / NC Health Choice

  1. Description of the service/program: 

The Health Check Program was developed in 1993 as a statewide initiative to improve Medicaid-enrolled children’s access to preventive health services. Health Check participants are eligible to receive comprehensive well child check ups, immunizations and also vision, hearing, and dental screening services on a regular basis throughout childhood. Medicaid covers any medically necessary diagnostic or treatment services needed to ameliorate or treat conditions identified through screening.   Along with Health Check, the NC Health Choice program provides low cost health insurance to uninsured children whose families cannot pay for private insurance and who do not qualify for Health Check (Medicaid).

Health Check Coordination Services
Health Check coordination services are offered to all Medicaid eligible children from birth to age 21.  A Health Check Coordinator is available to assist families in obtaining medical benefits, to serve as a link to community services, and to provide education concerning the advantages of regular preventative medical care.  Health Check Coordinators also follow-up on missed or delinquent Well-Child screenings, assist in obtaining transportation to medical appointments, and link families to doctors and other medical services.

  1. Who the service/program serves:   Both the Health Check and NC Health Choice programs are available to children and teens under 19 years of age (21 in some cases).

  • Well-Child Checkups
  • Medicines
  • Sick Visits
  • Dental Care
  • Immunizations (shots)
  • Hospital Care
  • Lab Tests
  • Counseling
  • Therapies
  • Surgeries
  • Medical Equipment and Supplies
  • Vision and Hearing Care

*Additional benefits may be available for children with special health care needs.

Transportation: 
If your children are enrolled in Health Check, transportation to medical appointments may be provided through the Department of Social Services.  If the children are enrolled in Health Choice, you must provide your own transportation.

e.    Applying for Health Check/NC Health Choice:


Applying for Health Check or NC Health Choice is easy.  Simply fill out and then submit an application.  You can get an application by contacting your local Department of Social Services or by calling the NC Family Health Resource Line at 1-800-367-2229….or download an application here.

Just mail or drop off the completed application at the Department of Social Services in the county where you live.  Please make sure that you answer all the questions completely and be sure to include all documents that the application requests (wage stubs, birth certificate, etc.).  This will help to make sure that your application is processed more quickly.
